03/09/1988Fission Stories #8 described a situation where a technician’s error caused two pumps to shut down, which lowered the reactor’s power level from 84 percent to 40 percent. This created unstable conditions in which the reactor power level began oscillating up and down until an automatic shut down was triggered at nearly 120 percent power.
05/21/1996Fission Stories #104 described how foam material being applied to seal cracks in the concrete inlet station inadvertently got into the water. The owner tried downplaying the significance by claiming that the material floated and therefore could not clog the pumps. Then the owner paid a $650,000 fine to the NRC for the excuse that didn’t float.
