03/09/1988   Fission Stories #8 described a situation where a technician’s error caused two pumps to shut down, which lowered the reactor’s power level from 84 percent to 40 percent. This created unstable conditions in which the reactor power level began oscillating up and down until an automatic shut down was triggered at nearly 120 percent power.
