1991  Fission Stories #75 described a misunderstanding that allowed workers to dismantle a component, carry it outside containment, apply a coating material to it, carry it back inside containment, and reinstall it. A procedure prohibited the application of this coating to equipment inside containment, but its uncertain wording enabled workers to exploit the loophole and get the offending coating inside containment.
